Does anyone know what a DEW776 manifold with 1/ 1/4 twin carbs was originally fitted to?

DEW776 sounds like an early job number not a part number.
If the manifold is not opened up for twin H4's then it most likely came from one of the small bore twin carb kits for a 998 that retained the HS2's.
